Sept. 16: A police vehicle arrives at Johns Hopkins Hospital in Baltimore after a shooting.
A man who became distraught as he was being briefed on his mother's condition at Johns Hopkins Hospital pulled a gun and shot the doctor Thursday, then killed his mother and himself in her room at the world-famous medical center, police said.
Baltimore police Commissioner Frederick H. Bealefeld III said the doctor had been talking to the 50-year-old man just after 11 a.m. when he "became emotionally distraught and reacted ... and was overwhelmed by the news of his mother's condition." The man was initially identified as Warren Davis, but police later said that may have been an alias.
Bealefeld said he did not know what the woman was being treated for at the world-class facility, known for its cancer research and treatment.
Davis then pulled a semiautomatic handgun from his waistband shot the doctor once in the abdomen, the commissioner said. The doctor collapsed outside room 873 of the Nelson building, where Davis' mother, Jean Davis, was being treated. Warren Davis then holed up inside his mother's room for more than two hours during a standoff with authorities.
When officers made their way to the eighth-floor room, they found Warren Davis dead on the floor and his mother dead in her hospital bed.
\Two Hopkins employees said the physician who was wounded is Dr. David B. Cohen, an assistant professor and orthopedic surgeon. The employees asked to remain anonymous because they are not authorized to discuss the matter. Police said the doctor underwent surgery and was expected to survive.
Michelle Burrell, who works in a coffee shop in the hospital lobby, said she was told by employees who were on the floor where the doctor was shot that the gunman was angry with the doctor's treatment of his mother.
"It's crazy," she said.
Hannah Murtaugh, 25, a first-year nursing student at the facility's nursing school, said her physiology class was put on lockdown. She said a classmate received a security alert text message from the school saying a gunman was on the Nelson building's eighth floor.
Her professor interrupted the lecture to let students know about the situation.
"They just kept telling us to stay away from the windows. We think there was a sniper located below our classroom," Murtaugh said.
"I was scared -- wondering if any of my friends or other students who had clinicals that day were on that floor, hoping the situation would be contained, trying to see what was going on while staying away from the windows."
She said security personnel helped keep everyone calm, making sure doors were locked and reviewing what could be done if the gunman was on the loose.
A small area of the hospital had been locked down before the gunman died, as about a dozen officers wearing vests and helmets and carrying assault weapons prepared to go into the hospital at midday.
The Nelson building is the main hospital tower. According to the Hopkins website, the eighth floor is home to orthopedic, spine, trauma and thoracic services.
The rest of the massive hospital, research and medical education complex remained open, including the emergency department.
With more than 30,000 employees, Johns Hopkins Medicine is among Maryland's largest private employers and the largest in Baltimore. The hospital has more than 1,000 beds and more than 1,700 full-time doctors.

Before we launched Advanced Practice Recruiters as a division of BSM, Inc. we were building the website under the name “Midlevel Recruiters.” However, as traffic started to come in from Google, Twitter, LinkedIn, and other places that we had began to develop a presence, we had a couple of complaints from Nurse Practitioners regarding the term “Mid-level.” We had represented enough NPs and PAs to know that this term wasn’t accepted by all, but we felt it was the most understood term for non-physician providers.
This being the case we scratched our heads and thought, “what should we call ourselves?” I called some Nurse Practitioner, Physician Assistant, and CRNA friends of mine to ask what they thought. Most of them said they weren’t offended by the term Mid-Level, but that they preferred to be called by their official title, respectively. Well we ruled out the name, “Physician Assistant Nurse Practitioner CRNA Clinical Nurse Specialist Recruiters,” and looked around for more answers.
When we came across the AANP position paper on the term Midlevel, found HERE, and decided that Midlevel Recruiters wasn’t going to work long-term. The position paper recommended the terms “independently licensed providers, primary care providers, healthcare professionals, and clinicians. Out of these we felt the one with the most relevance was “clinicians,” but we quickly learned most non-practitioners didn’t recognize this term. We ended up deciding on the name, “Advanced Practice Recruiters,” borrowing the term from the title, “Advanced Practice Nurse,” that includes NPs, CRNAs, and CNSs.
To further our understanding on what the community of healthcare professionals thought about this term we included a survey when we launched the new website under the Advanced Practice Recruiters name. To date we have had 110 responses.
Luckily the term, “Advanced Practitioner,” is leading the way, but surprising to us the term, “Mid-level,” is not far behind. What are your thoughts? Chime in HERE as we hold this survey open until the end of the year 2010.

Foreign-Born Doctors Give Equal Care in U.S.
Patients treated by foreign-born doctors who trained in other countries fare just as well as people treated by doctors educated in the United States, a new study has found.
But the results are not as good when the doctor is an American who went to medical school overseas and then returns to practice, the researchers determined. In that situation, patients with heart disease have longer hospital stays and slightly higher death rates.
Graduates of foreign medical schools now make up a quarter of all the practicing doctors in the United States. In order to qualify here, they have to pass a series of rigorous exams and complete residency training. About one-fifth of the foreign-trained doctors in the United States are Americans who studied abroad, often at medical schools in the Caribbean. Most foreign-born doctors in the United States come from India or Pakistan and initially studied medicine in those countries.
The study is being published Tuesday in the August issue of the journal Health Affairs. The first author is John J. Norcini, president of the Foundation for Advancement of International Medical Education and Research, in Philadelphia.
The authors of the study offered two possible reasons the Americans who went to foreign medical schools might not perform as well as doctors trained in the United States, or as well as foreign-born doctors. One is that many of the Americans who study medicine elsewhere do so because their grades and test scores were too low to get into medical school in the United States — so they may be less capable in the first place. Another possibility is that some of the overseas medical schools Americans attend may not be up to par.
There is a doctor shortage in the United States, and foreign doctors have stepped into the breach, particularly in specialties like internal medicine and family practice, which many American students have turned away from in favor of more lucrative specialties like cardiology. In response to the doctor shortage, medical schools in the United States have begun to expand their class sizes, and some new schools have opened. But the number of residency spots is not increasing, so new American graduates may start squeezing out the foreign-born doctors when it comes time to start residencies.
The authors say their findings offer a cautionary note: if schools in the United States lower their standards to fill new spots and begin admitting the kind of American applicants who have been going overseas to study, they may start turning out less competent doctors.
Dr. Norcini said there had been concern about the competence of foreign-trained doctors, based in part on reports in the 1990s of lower test scores and performance ratings. But his study noted that “by the mid-1990s, international medical graduates were outperforming U.S. graduates” on tests in internal medicine.
The researchers set out to evaluate doctors by assessing the health of their patients. They analyzed records from 244,153 hospitalizations in Pennsylvania from 2003-06. All the patients had congestive heart failure or had suffered heart attacks, conditions that are considered a good gauge of the quality of medical care.
The patients were treated by 6,113 doctors. As in the rest of the country, about three-quarters of them were born and trained in the United States. The rest had trained in other countries, and most were foreign-born; about 400 were Americans who had trained overseas.
The patients of foreign-born international graduates had the lowest death rate, 5 percent, and the patients of American doctors trained overseas had the highest death rate, 5.8 percent. Patients of the American born-and-trained doctors fell in the middle, with 5.5 percent.

General Information
Q. What is a Physician Assistant (PA)?
A. Physician assistants are health care professionals licensed to practice medicine with physician supervision. PAs employed by the federal government are credentialed to practice. As part of their comprehensive responsibilities, PAs conduct physical exams, diagnose and treat illnesses, order and interpret tests, counsel on preventive health care, assist in surgery, and in virtually all states can write prescriptions. Within the physician-PA relationship, physician assistants exercise autonomy in medical decision making and provide a broad range of diagnostic and therapeutic services. A PA's practice may also include education, research, and administrative services.
PAs are trained in intensive education programs accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) .
Because of the close working relationship the PAs have with physicians, PAs are educated in the medical model designed to complement physician training. Upon graduation, physician assistants take a national certification examination developed by the National Commission on Certification of PAs in conjunction with the National Board of Medical Examiners. To maintain their national certification, PAs must log 100 hours of continuing medical education every two years and sit for a recertification every six years. Graduation from an accredited physician assistant program and passage of the national certifying exam are required for state licensure.
Q. How did the Physician Assistant profession begin?
A. In the mid-1960s, physicians and educators recognized there was a shortage and uneven distribution of primary care physicians. To expand the delivery of quality medical care, Dr. Eugene Stead of the Duke University Medical Center in North Carolina put together the first class of PAs in 1965. He selected Navy corpsmen who received considerable medical training during their military service and during the war in Vietnam but who had no comparable civilian employment. He based the curriculum of the PA program in part on his knowledge of the fast-track training of doctors during World War II.
Q. What does "PA-C" stand for? What does the "C" mean?
A. Physician assistant-certified. It means that the person who holds the title has met the defined course of study and has undergone testing by the National Commission on Certification of Physician Assistants (NCCPA). The NCCPA is an independent organization, and the commissioners represent a number of different medical professions. It is not a part of the PA professional organization, the American Academy of Physician Assistants (AAPA). To maintain that "C" after "PA", a physician assistant must log 100 hours of continuing medical education every two years and take the recertification exam every six years.
Q. How is a Physician Assistant educated?
A. Physician assistants are educated in intensive medical programs accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA). The average PA program curriculum runs approximately 26 months. There are currently more than 130 accredited programs. All PA programs must meet the same ARC-PA standards.
Because of the close working relationship PAs have with physicians, PAs are educated in a medical model designed to complement physician training. PA students are taught, as are medical students, to diagnose and treat medical problems.
Education consists of classroom and laboratory instruction in the basic medical and behavioral sciences (such as anatomy, pharmacology, pathophysiology, clinical medicine, and physical diagnosis), followed by clinical rotations in internal medicine, family medicine, surgery, pediatrics, obstetrics and gynecology, emergency medicine, and geriatric medicine.
A PA's education doesn't stop after graduation, though. PAs are required to take ongoing continuing medical education classes and be retested on their clinical skills on a regular basis. A number of postgraduate PA programs have also been established to provide practicing PAs with advanced education in medical specialties.
Q. What are the prerequisites for applying to a PA program?
A. PA programs look for students who have a desire to study, work hard, and to be of service to their community. Most physician assistant programs require applicants to have previous health care experience and some college education. The typical applicant already has a bachelor's degree and approximately 4 years of health care experience. Commonly nurses, EMTs, and paramedics apply to PA programs. Check with PA educational programs of interest to you for a list of their prerequisites.

A. Physician assistants (PAs) are found in all areas of medicine. They practice in the areas of primary care medicine - that is family medicine, internal medicine, pediatrics, and obstetrics and gynecology -- as well in surgery and the surgical subspecialties.
Physician assistants receive a broad education in medicine. Their education is ongoing after graduation through continuing medical education requirements and continual interaction with physicians and other health care providers.
Q. Where do PAs "draw the line" as far as what they can treat and what a physician can treat?
A. What a physician assistant does varies with training, experience, and state law. In addition, the scope of the PA's practice corresponds to the supervising physician's practice. In general, a physician assistant will see many of the same types of patients as the physician. The cases handled by physicians are generally the more complicated medical cases or those cases which require care that is not a routine part of the PA's scope of work. Referral to the physician, or close consultation between the patient-PA-physician, is done for unusual or hard to manage cases. Physician assistants are taught to "know our limits" and refer to physicians appropriately. It is an important part of PA training.
Q. Can PAs prescribe medications?
A. Forty-eight states, the District of Columbia, and Guam have enacted laws that authorize PA prescribing. In California, PA prescriptions are referred to as written prescription transmittal orders.
Q. What do physicians think about Physician Assistants?
A. Most physicians who have worked with physician assistants like having PAs on staff. The American Medical Association, the American College of Surgeons, the American Academy of Family Physicians, the American College of Physicians, and other national medical organizations support the physician assistant profession by actively supporting the PA certifying commission and the PA program accrediting agency.
Studies done by the Federal Government have shown that PAs, working with the supervision of physicians, provide care that is comparable to physician care. The Eighth Report to the President and Congress on the Status of Health Personnel in the United States (released in 1992) states, "Physician assistants have demonstrated their clinical effectiveness both in terms of quality of care and patient acceptance."
Q. What is the working relationship between a physician and a physician assistant?
A. The relationship between a PA and the supervising physician is one of mutual trust and respect. The physician assistant is a representative of the physician, treating the patient in the style and manner developed and directed by the supervising physician. The physician and PA practice as members of a medical team. In 1995, the American Medical Association developed suggested guidelines for how physicians and PAs should work as a team in the delivery of medical care.

Adopted by the AMA House of Delegates, June 1995
Reflecting the comments from the American Academy of Physician Assistants, separate model guidelines for Physician/Physician Assistants practice have been developed. These are based on the unique relationship of Physician Assistants who recognize themselves as agents of physicians with respect to delegated medical acts, and legal responsibilities. They are consistent with the existing AMA policies concerning Physician Assistants cited in this report. In all settings, Physician Assistants recognize physician supervision in the delivery of patient care. The suggested guidelines reflect those as follows:
Q. What's the difference between a PA and a physician?
A. Physician assistants are educated in the "medical model"; in some schools they attend many of the same classes as medical students.
One of the main differences between PA education and physician education is not the core content of the curriculum, but the amount of time spent in formal educationl. In addition to time in school, physicians are required to do an internship, and the majority also complete a residency in a specialty following that. PAs do not have to undertake an internship or residency.
A physician has complete responsibility for the care of the patient. PAs share that responsibility with the supervising physicians.
Q. What is the American Academy of Physician Assistants (AAPA)?
A. The AAPA is the only national professional society to represent all physician assistants in every area of medicine. Founded in 1968, the academy has a federated structure of 57 chartered chapters representing PAs in all 50 states, the District of Columbia, Guam, and the federal services.
AAPA's mission is to provide quality, cost-effective, and accessible health care as well as to support the professional and personal development of PAs. The AAPA pursues these goals through government relations and public education programs, research and data collection efforts, and continuing education activities.
The Academy's policies are set by the House of Delegates, which meets once a year, and implemented by the Board of Directors. The House of Delegates is made up of representatives from the chartered chapters, specialty organizations, the Student Academy, and the Association of PA Programs. A. Here is an interesting article about funny things that people have written on their resume. I also want to urge any medical providers NOT to put their social security number, DEA number, medical licensure number, or even any references on your resume/CV. Why you ask? I’ll tell you. NEVER give your social out for any reason to someone you don’t know. It can lead to identity theft and none of us want to go through that. The hiring party will ask for it when it is appropriate. DEA number? Same thing. I spoke with the Drug Enforcement Agency personally and asked them about this. They said to guard your DEA number as closely as you would your Social Security Number. Same with your medical licensure number. Here is where I let you in on a little industry secret. When I see references on a resume or CV, my first instinct is to call them and see if they are looking for a new job. Granted, I do not call them because I don’t want to violate the trust that I have with the candidate, but I can promise you that 90% of the recruiters that you send your information will do this. You can make a lot of colleagues unhappy if they are listed as references and they get a ton of phone calls from random recruiters. Just something to think about. When a potential employer needs your references, they will ask for them. Don’t volunteer them. My advice would be to put “References Available Upon Request” at the bottom. And now for the article…

We are experiencing an unprecedented hiring demand for Advanced Practice Providers. Throughout American history there has always been a primary care provider shortage, however in recent history this shortage has escalated into a health care crisis. This map is a visual representation of the areas with the greatest demand yet the least supply. For a geographic area to be represented on this map there must be at least 10 job openings available and a supply/demand ratio grater than100%. I have a client who is currently using our help to find a rockstar candidate. I found a rockstar for them, but that is not why I am writing this. I’m writing this because my client-we’ll call her Sheridee-had previously interviewed four candidates and only liked one of them. The problem with the one Sheridee liked was that he didn’t wear a suit to the interview. Who does that nowadays?? The guy showed up in slacks and a polo, which is good for a Kindergarten graduation maybe, but not suitable (no pun intended) for a job interview. Even though you are in the medical field, it is still business. In business you have to dress for success and a job interview is no different. They are going to pay you for your services, and your first impression is crucial. Give a homeless man a bath, shave his beard, and put him in a suit and he could probably get a well-paying job. Take a fortune 500 CEO and have him show up to an interview in jeans and a t-shirt and I don’t think anyone would hire him. Some of the other intangibles are: bring a copy of your CV/Resume on CARD STOCK. Don’t print it on plain white copier paper. It looks limp and unprofessional. It will also blend in well with the other resumes that have been faxed in for the position. You want to stick out in an interview as someone they won’t forget. You want the same for your resume. Be prepared to ASK questions. You aren’t there just to answer them. Asking questions about the position and practice shows that you are attentive and engaged. Even having a list of questions that were written before the interview is good. Doing this will also prevent you from leaving the interview and forgetting to ask about the 401k, HSA, or any other question you might be interested in knowing. I would also recommend doing some research on the facility. Google is your best friend in this situation. You can google email addresses, first and last names, and even get access to past and pending litigation against the provider. It will also give you a leg up if you mention the article you read about their practice in XYZ publication during the interview. We live in the information age, but the information is of no value unless you use it! Another tip I would suggest is googling yourself as well as your own email address. I once spoke to a surgeon who was looking for a job. I googled his email address and nothing came up. Then I googled his email address without the “@xyzmail.com” at the end of it and his profile came up on a married couple’s XXX picture swapping site. If you don’t think that your potential employer is doing this, then think again. They’ll look up your facebook, linkedin, myspace, etc. to see what kind of web presence you have. If your name is Bill Jones then it will be a little more difficult. If your name is Samir Nahinanaja then it will be pretty easy to find you. Look yourself up online before you have to start doing some explaining about the picture of you passed out at your best friend’s bachelorette party. Most of what I’m telling you here is common sense, but you’d be surprised at how many people aren’t doing it. Be prepared, be wise, and be employed.

CHICAGO — A nurse may soon be your doctor. With a looming shortage of primary care doctors, 28 states are considering expanding the authority of nurse practitioners. These nurses with advanced degrees want the right to practice without a doctor's watchful eye and to prescribe narcotics. And if they hold a doctorate, they want to be called "Doctor."
For years, nurse practitioners have been playing a bigger role in the nation's health care, especially in regions with few doctors. With 32 million more Americans gaining health insurance within a few years, the health care overhaul is putting more money into nurse-managed clinics.
Those newly insured patients will be looking for doctors and may find nurses instead.
The medical establishment is fighting to protect turf. In some statehouses, doctors have shown up in white coats to testify against nurse practitioner bills. The American Medical Association, which supported the national health care overhaul, says a doctor shortage is no reason to put nurses in charge and endanger patients.
Nurse practitioners argue there's no danger. They say they're highly trained and as skilled as doctors at diagnosing illness during office visits. They know when to refer the sickest patients to doctor specialists. Plus, they spend more time with patients and charge less.
"We're constantly having to prove ourselves," said Chicago nurse practitioner Amanda Cockrell, 32, who tells patients she's just like a doctor "except for the pay."
On top of four years in nursing school, Cockrell spent another three years in a nurse practitioner program, much of it working with patients. Doctors generally spend four years in undergraduate school, four years in medical school and an additional three in primary care residency training.
Medicare, which sets the pace for payments by private insurance, pays nurse practitioners 85 percent of what it pays doctors. An office visit for a Medicare patient in Chicago, for example, pays a doctor about $70 and a nurse practitioner about $60.
The health care overhaul law gave nurse midwives, a type of advanced practice nurse, a Medicare raise to 100 percent of what obstetrician-gynecologists make — and that may be just the beginning.
States regulate nurse practitioners and laws vary on what they are permitted to do:
_ In Florida and Alabama, for instance, nurse practitioners are barred from prescribing controlled substances.
_ In Washington, nurse practitioners can recommend medical marijuana to their patients when a new law takes effect in June.
_ In Montana, nurse practitioners don't need a doctor involved with their practice in any way.
_ Many other states put doctors in charge of nurse practitioners or require collaborative agreements signed by a doctor.
_ In some states, nurse practitioners with a doctorate in nursing practice can't use the title "Dr." Most states allow it.
The AMA argues the title "Dr." creates confusion. Nurse practitioners say patients aren't confused by veterinarians calling themselves "Dr." Or chiropractors. Or dentists. So why, they ask, would patients be confused by a nurse using the title?
The feud over "Dr." is no joke. By 2015, most new nurse practitioners will hold doctorates, or a DNP, in nursing practice, according to a goal set by nursing educators. By then, the doctorate will be the standard for all graduating nurse practitioners, said Polly Bednash, executive director of the American Association of Colleges of Nursing.
Many with the title use it with pride.
"I don't think patients are ever confused. People are not stupid," said Linda Roemer, a nurse practitioner in Sedona, Ariz., who uses "Dr. Roemer" as part of her e-mail address.
What's the evidence on the quality of care given by nurse practitioners?
The best U.S. study comparing nurse practitioners and doctors randomly assigned more than 1,300 patients to either a nurse practitioner or a doctor. After six months, overall health, diabetes tests, asthma tests and use of medical services like specialists were essentially the same in the two groups.
"The argument that patients' health is put in jeopardy by nurse practitioners? There's no evidence to support that," said Jack Needleman, a health policy expert at the University of California Los Angeles School of Public Health.
Other studies have shown that nurse practitioners are better at listening to patients, Needleman said. And they make good decisions about when to refer patients to doctors for more specialized care.
The nonpartisan Macy Foundation, a New York-based charity that focuses on the education of health professionals, recently called for nurse practitioners to be among the leaders of primary care teams. The foundation also urged the removal of state and federal barriers preventing nurse practitioners from providing primary care.
The American Medical Association is fighting proposals in about 28 states that are considering steps to expand what nurse practitioners can do.
"A shortage of one type of professional is not a reason to change the standards of medical care," said AMA president-elect Dr. Cecil Wilson. "We need to train more physicians."
In Florida, a bill to allow nurse practitioners to prescribe controlled substances is stalled in committee.
One patient, Karen Reid of Balrico, Fla., said she was left in pain over a holiday weekend because her nurse practitioner couldn't prescribe a powerful enough medication and the doctor couldn't be found. Dying hospice patients have been denied morphine in their final hours because a doctor couldn't be reached in the middle of the night, nurses told The Associated Press.
Massachusetts, the model for the federal health care overhaul, passed its law in 2006 expanding health insurance to nearly all residents and creating long waits for primary care. In 2008, the state passed a law requiring health plans to recognize and reimburse nurse practitioners as primary care providers.
That means insurers now list nurse practitioners along with doctors as primary care choices, said Mary Ann Hart, a nurse and public policy expert at Regis College in Weston, Mass. "That greatly opens up the supply of primary care providers," Hart said.
But it hasn't helped much so far. A study last year by the Massachusetts Medical Society found the percentage of primary care practices closed to new patients was higher than ever. And despite the swelling demand, the medical society still believes nurse practitioners should be under doctor supervision.
The group supports more training and incentives for primary care doctors and a team approach to medicine that includes nurse practitioners and physician assistants, whose training is comparable.
"We do not believe, however, that nurse practitioners have the qualifications to be independent primary care practitioners," said Dr. Mario Motta, president of the state medical society.
The new U.S. health care law expands the role of nurses with:
_ $50 million to nurse-managed health clinics that offer primary care to low-income patients.
_ $50 million annually from 2012-15 for hospitals to train nurses with advanced degrees to care for Medicare patients.
_ 10 percent bonuses from Medicare from 2011-16 to primary care providers, including nurse practitioners, who work in areas where doctors are scarce.
_ A boost in the Medicare reimbursement rate for certified nurse midwives to bring their pay to the same level as a doctor's.
The American Nurses Association hopes the 100 percent Medicare parity for nurse midwives will be extended to other nurses with advanced degrees.
"We know we need to get to 100 percent for everybody. This is a crack in the door," said Michelle Artz of ANA. "We're hopeful this sets the tone."
In Chicago, only a few patients balk at seeing a nurse practitioner instead of a doctor, Cockrell said. She gladly sends those patients to her doctor partners.
She believes patients get real advantages by letting her manage their care. Nurse practitioners' uphill battle for respect makes them precise, accurate and careful, she said. She schedules 40 minutes for a physical exam; the doctors in her office book 30 minutes for same appointment.
Joseline Nunez, 26, is a patient of Cockrell's and happy with her care.
"I feel that we get more time with the nurse practitioner," Nunez said. "The doctor always seems to be rushing off somewhere."

As the nation weighs in on the passing of heath care reform, conflicting opinions and perspectives are running wildly through the medical community. Advanced for Nurse Practitioners tried to gain insight to the thoughts of NPs by asking some leaders to share their perspectives. Here are some of the perspectives that caught my attention:
Susan Wysocki, NP, president and CEO of Nurse Practitioners in Women's Health said,
“If anything, the new healthcare reform legislation underscores the importance of recognizing and supporting the role that nurse practitioners play in the healthcare system. It is one thing for more people to have health insurance, it is another to make sure that people can access health services. Access to NP care will be a critical component to meet the increased demand for services, particularly as they relate to keeping Americans healthy and thereby keeping costs down.”
Patricia Kappas-Larson, NP, president of the Gerontological Advanced Practice Nurses Association, says that by the time the details are hammered out, NPs may be excluded from reimbursement provisions that require an increase in Medicaid payments to primary care physicians:
“Our concern as regarding this move as part of healthcare reform is primarily what it means with regards to recognition of nurse practitioners and their role in the provision of primary care. Not only does this represent an issue of reimbursement, but also the associated language excludes nurse practitioners and would indicate physicians only will be deemed primary care providers. This is an issue for our practice but most importantly to our patients who deserve and desire the primary care we deliver and has been validated with research is of high quality. Patients need and want access; access requires acknowledgement and acknowledgement should lead to appropriate reimbursement for services rendered.”

Outside of the majority of the attributes that cause one to be for or against this bill, as a Recruiter of Nurse Practitioners, I have concerns for the delivery of healthcare to all individuals. The United States has been short of Primary Care providers for decades and now we are entering a time when Baby-boomers are retiring, aging, and seeking healthcare. If suddenly everyone has medical benefits and begins seeking healthcare the questions that comes to my mind is, “Who is going to see all of these new patients?”
As Advance for Nurse Practitioners states, “The biggest change, though -- health insurance for 32 million more U.S. residents over the next 10 years -- means an immediate need for more primary care providers. Although the picture of the new healthcare reform act won't be clear for quite a while, what is clear now is that nurse practitioners have been and will be an indispensable part of the healthcare system of this country.”
Tweet doctors? Really!!? Could your doctor start communicating with you via Twitter. I don’t think so…
However, a poll conducted by Case Western University showed 59% of percent of doctors who participated in the poll said that Twitter would become an integral part of the way doctors communicate with patients and other medical professionals.

Doctors don't generally use Twitter to give medical advice but to guide the public to reputable sources of information or share breaking medical news, said Kevin Pho, MD, in a post on his medical blog, www.KevinMD.com
Pho conducted a similar poll in Aug. 2009, asking medical professionals how they used Twitter. He found that about 45 percent did not use the service at all. Thirty-six percent said they used it to share and keep up with news and information and only 1.5 percent said they used it to connect with patients.
